As part of the normal operation of our services we collect and, in some cases, disclose information about you. We do not currently accept any information submitted to or posted at our website from children under the age of 13. No information should be submitted to or posted at our website by children under 13 years of age without the consent of their parent or legal guardian. Your Registration and Other Personal Information. To fully use our services, you will need to obtain a user ID and password from your treating healthcare professional. You then need to register using our online registration form, where you can order products, register for programs and services, submit forms, make requests to receive materials, participate in online programs, fill out surveys, and communicate with Lifestyle Innovations staff members and fellow program participants. The personal information collected on these pages may include a variety of information including your name, contact details, and credit card information. If you join an online or by-phone program, it may also include your weight, your physical activities, various demographic, medical, and physical parameters, and details about the foods you eat. Unless you give us permission to do so, we will not sell, share, license, trade or rent your Personally Identifiable Information other than as specified in this Privacy Policy.


Your Usage of Our Site. We automatically track certain information about you based upon your use of our site. We use this information to conduct internal research on our users' demographics, interests, and behavior to better understand and serve our users. This information is compiled and analyzed on an aggregate basis. Healthcare Professionals. Your treating healthcare professional will have direct access to your surveys, health and weight management status. WE will also give your healthcare professional relevant updates about ongoing promotions and any new communications relevant to your use of the site.

We may disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose us to legal liability.
 
Advertisers. We disclose aggregate information about our users to advertisers and for other marketing and promotional purposes. However, we do not disclose any personally identifying information to any of these entities.
 
LIwellness.com Site. Throughout the Site, you are identified by your User ID. You will be assigned a User ID and password from your healthcare professional, who will also have access to your information. Unless you select a unique User ID, our system automatically sets your User ID as your e-mail address. In this situation, where you are identified on the Site by your User ID, you disclose your e-mail address to other users and third parties. As a result, other users and third parties might send you unsolicited e-mail and be able to personally identify all of your activities on the Site. Although our pages are coded with robot exclusion headers, others may attempt to automatically collect your e-mail address from our site. We describe how to change your User ID below. Even if your User ID is not your e-mail address, your User ID can be used to identify you and your activities throughout the Site.
 
Third Party Suppliers. We may use third party suppliers to assist us in providing the services available on our web site. Whenever a third party supplier desires to collect information about you, you will be notified of the involvement of the third party supplier, and you will never be required to disclose and information to the third party supplier.
